Your employer has asked us to create a confidential report to check how you are feeling and evaluate wellbeing.

Please take just a few moments to answer these questions. We will not ask you to share any personal contact information with us and all identifiable personal data will be removed and not stored.

Your job and your home life inevitably affect your wellbeing and mental health even without the pressures linked to COVID-19. Whether you’re in a high-pressure position; working irregular / long hours; or you feel unsupported and are disliking remote working, these can all negatively impact your wellbeing for many reasons.

We’ve pooled the best resources from Mental Health First Aid’s Take 10, Mental Health Foundation’s good mental health survey, and the NHS Mood zone self-assessment to create a checklist which may help you identify whether and where you’re struggling, or simply if you are a little bit stuck; and how well@work can help!
